Address Confirmation

Your child has been selected by Dr. Peirson for testing of folate receptor autoantibodies. While we have your address on file as part of your child’s medical record, we ask that you fill this out in the event that we do not have a current address for you.

Your child's name and address will be sent to the director of Religen Labs, who will then find phlebotomy centers in your area that can process the blood draw. This test cannot be processed through most Labcorp, Quest Diagnostics or hospital labs.
